Fat, drunk and stupid

How old is this phrase and what is its origin? I heard it for the first time about 6 years ago, but I am sure it is much older than that.

I first heard it in the 1970s, spoken by a gynecologist. He was talking about himself. At least, I think he was.

And then there's the line from "Animal House" . Dean Vernon Wormer: "Fat, drunk, and stupid is no way to go through life, son."
