Is there a question here? The sentence, while not exactly a proverb, could be considered an apothegm, I suppose. If repeated by enough people, it could become a saying. But first rewrite it to say: "An ulcer is the surest sign of executive success." Supposedly an ulcer is frequently the result of emotional stress, as in having to worry a lot about the company's affairs and perhaps having to fire a lot of people. In this sentence "executive success" has to mean "success as an executive (or official) of a business or other organization." Is this what you wanted to know? SS
