It is most famously associated with the Congressional Medal of Honor (for the military) and similar awards bestowed police and firefighters for valor "above and beyond the call of duty."

The Medal of Honor dates back to the Civil War, but I would not be a bit surprised to find the phrase was lifted from prior awards for heroism.
