: A good head could also include a high level of intelligence.

Not just any old kind of intelligence, though. "Having a good head on one's shoulders" implies practicality, like someone who makes a smart play in a baseball game or the little pig who built his house of bricks. We wouldn't usually talk that way about a poet.
