In West Virginia we meant that literally. If the creek rose, people might not be able to cross and do whatever. I have a friend from the South who used to say if we don't get hit by a bus, we would do this or that. And I've heard, said of someone in bad shape, "You look like you've been hit by a Mack truck." And someone who was broad was said to look like a Mack truck with both doors open.
