It means "How's it going?" when you're in a mischievous or irreverent mood. You know what "it" is, I hope. Use the phrase when speaking to a woman only when the irony would be understood and appreciated. By the way, this is a pretty widely known phrase in the U.S., at least in the circles I travel in.
