It's actually "twelve step program" and it originated with Alcoholics Anonymous in 1935. The meaning is pretty simple. The first step toward recovery from alcoholism is to admit you have a problem, the second is to admit you can't solve this problem by yourself, and so on. The steps get harder as you go, of course.
