It is used to convey the idea that the bone is not whole and you can feel the separate pieces under the skin. It refers to the sound it makes when the indivdual pieces are rubbing together and make a grinding sound. As far as a substitute for this word? How about just saying "one can feel bone fragments under the skin on the forehead"?
