I can't find the origin of "heavy heavy", but the ritual is mentioned in the book American Children's Folklore (Simon J. Bronner). It is the title of Chapter 8. According to Bronner, it goes:

Heavy heavy hangs over thy head. What do you wish for this person, with a bump on the head?
