Long ago and far away

The phrase "long ago and far away" was used at the beginning of the first Star Wars movie. But I've also recently found it in my Dad's jazz record collection as a title of an LP from 1950. Does the phrase have a known origin? Thanks. - Patty

The exact Star Wars intro is "A Long Time Ago in a Galaxy Far, Far Away...."

All I found was that "Long Ago (And Far Away)" is a musical number from the 1944 Hollywood musical "Cover Girl" with the music by Jerome Kern,lyrics by Ira Gershwin and sung by Perry Como.

Isn't this the standard fairy tale opening?
